1. As a Revive Med Spa & Aesthetics VIP member, your payments will be collected monthly. Complete payment for products and services rendered must be made at the time of treatment. Membership discounts cannot be combined with other offers or discounts

2. All products and services offered at Revive Med Spa & Aesthetics are subject to change. Members will be notified of any changes to membership pricing, terms, or conditions at least 30 days in advance.

3. You may cancel your membership at any time after 3 months. Memberships cannot be canceled within the first 3 months. You may contact Revive Med Spa & Aesthetics to cancel. Please note that we do not allow for pausing of Memberships.

4. Memberships at Revive Med Spa & Aesthetics are non-transferable and must be redeemed by the member only.

5. You must be 18 years of age or older to sign up for a membership.

6. Revive Med Spa & Aesthetics reserves the right to review memberships and deny, suspend, or cancel your membership at any time if the terms of this membership agreement are not met.
